Thriving Communities Trafford was a 12-month Social Prescribing pilot project which ran in Old Trafford and Stretford in 2021-22, funded by Arts Council England and the National Academy for Social Prescribing. The project was led by Stretford Public Hall, with St John’s Centre as a Core Partner.

Eight wellbeing activities – from fitness coaching to telephone music sessions – were developed between local community groups and patients who were referred to them by local Social Prescribing Link Workers.

As part of the project, valuable lessons were learnt about how small community groups can be supported to provide activities for Social Prescribing. To find out more, you can download the Thriving Communities Final Evaluation Report or Executive Summary.
